False anodontia

Overview

False aneurysm, also known as a pseudoaneurysm, is a localized dilation of a blood vessel wall caused by a defect in the intima and media, allowing blood to leak into the surrounding tissue without forming a true encapsulated hematoma. It often results from trauma, iatrogenic injury, or infection 12567101213.

Diagnosis

  • Clinical Presentation: Localized swelling, pulsatile mass, pain, and sometimes rupture 13471112.
  • Imaging: Ultrasound, CT, MRI, and angiography are crucial for diagnosis, showing a characteristic fluid collection with a pulsating signal 124561012.
  • Differential Diagnosis: Mesenchymal tumors, abscesses, hematoma, and other vascular malformations 213.
  • Management

  • Surgical Repair: Primary repair or resection with end-to-end anastomosis is often necessary, especially for larger aneurysms or those at risk of rupture 1457.
  • Endovascular Techniques: Embolization with coils or gel foam can be effective for smaller pseudoaneurysms 9.
  • Vascular Control: Initial control with digital compression or tourniquets may be required before definitive repair 57.
  • Antibiotics: If infection is suspected or present, appropriate antibiotic therapy should be initiated 6.
  • Special Populations

  • Pediatrics: False aneurysms can occur due to minor trauma or iatrogenic injury; prompt diagnosis and surgical intervention are critical to prevent complications like hemorrhage 3512.
  • Comorbidities: In cases involving stents or other vascular interventions, infection poses a significant risk requiring vigilant monitoring and management 6.
  • Key Recommendations

  • Prompt Imaging: Utilize ultrasound, CT, or MRI for early diagnosis of false aneurysm to guide management 124561012 (Evidence: Strong).
  • Surgical or Endovascular Repair: For symptomatic or large pseudoaneurysms, surgical repair or endovascular embolization should be considered to prevent rupture 14579 (Evidence: Moderate).
  • Control Bleeding: Apply immediate vascular control measures such as digital compression before definitive surgical intervention 57 (Evidence: Expert opinion).
  • Antibiotic Therapy for Infection: In cases of infected false aneurysms, initiate broad-spectrum antibiotics tailored to culture results 6 (Evidence: Moderate).
  • Close Monitoring in Special Populations: Especially in pediatric patients and those with vascular implants, close monitoring and early intervention are crucial 356 (Evidence: Expert opinion).
