Ovarian failure

Last edited: 4/14/2026

Overview

Premature ovarian failure (POF) is characterized by the cessation of ovarian function before age 40, leading to amenorrhea, infertility, and often hormonal imbalances such as amenorrhea and elevated gonadotropin levels. 123

Diagnosis

  • Amenorrhea for at least 3 consecutive cycles
  • Elevated follicle-stimulating hormone (FSH) and luteinizing hormone (LH) levels on at least two occasions 4 weeks apart
  • Low estradiol levels
  • Exclusion of other causes of secondary amenorrhea 2
  • Management

  • Hormone Replacement Therapy (HRT): Estrogen and progesterone to manage symptoms and prevent osteoporosis (specific doses not detailed) 2
  • Low-Intensity Pulsed Ultrasound (LIPUS): Emerging therapy for promoting ovarian function and tissue regeneration (specific protocols not detailed) 1
  • Ovarian Biopsy: Minilaparoscopic approach under conscious sedation can be feasible and effective compared to traditional laparoscopy for diagnostic purposes 4
  • Special Populations

  • Pregnancy: Cyclophosphamide use should be avoided in women with incomplete family planning due to risk of POF 2
  • Comorbidities: No specific pediatric or elderly considerations mentioned in the abstracts 34
  • Key Recommendations

  • Avoid cyclophosphamide in women with incomplete family planning to prevent premature ovarian failure (Evidence: Strong) 2
  • Consider low-intensity pulsed ultrasound as a potential adjunctive therapy for promoting ovarian function in POF (Evidence: Weak) 1
  • Minilaparoscopic ovarian biopsy under conscious sedation may be a feasible alternative to traditional laparoscopy for diagnostic procedures in POF (Evidence: Moderate) 4
