Acute COVID-19

Last edited: 4/14/2026

Overview

Post-acute COVID-19 syndrome (PACS), also known as long COVID, involves persistent symptoms such as fatigue, cognitive impairment, and gastrointestinal issues affecting approximately 10% of recovered individuals 1. These symptoms can persist for months and impact multiple organ systems, leading to significant morbidity 38.

Diagnosis

  • Key Symptoms: Fatigue, cognitive impairment, breathlessness, cough, myalgia, arthralgia, sleep disturbance, and cognitive impairment 8.
  • Assessment: Comprehensive evaluation including history, physical examination, and targeted organ-specific assessments 4.
  • Persistence Criteria: Symptoms lasting ≥12 weeks post-acute infection 5.
  • Diagnostic Tests: No specific diagnostic tests; often relies on exclusion of other conditions 4.
  • Management

  • First-Line Treatments: Symptomatic management including physical therapy, cognitive behavioral therapy, and lifestyle modifications 9.
  • Adjunctive Treatments: Multidisciplinary rehabilitation approaches tailored to individual needs, particularly in geriatric populations 9.
  • Pharmacological Interventions: Limited specific drug recommendations; focus on symptom relief (e.g., analgesics for myalgia, sleep aids) 8.
  • Monitoring: Regular follow-up to assess symptom progression and impact on quality of life 10.
  • Special Populations

  • Elderly: Geriatric rehabilitation guidance emphasizes tailored rehabilitation programs to address functional decline 9.
  • Comorbidities: Patients with pre-existing conditions may require more intensive monitoring and multidisciplinary care 5.
  • Key Recommendations

  • Comprehensive Assessment: Conduct thorough evaluations including symptom assessment across multiple organ systems 4 (Evidence: Strong).
  • Multidisciplinary Rehabilitation: Implement multidisciplinary rehabilitation strategies for effective management, especially in geriatric patients 9 (Evidence: Moderate).
  • Symptom-Based Management: Focus on symptomatic treatment and lifestyle interventions to improve quality of life 8 (Evidence: Moderate).
  • Regular Follow-Up: Schedule regular follow-up appointments to monitor symptom persistence and adjust management plans accordingly 10 (Evidence: Expert opinion).
