Congenital junctional ectopic tachycardia

Last edited: 4/22/2026

Overview

Congenital junctional ectopic tachycardia (JET) is characterized by an incessant tachycardia with normal QRS morphology and atrioventricular dissociation, typically presenting in infancy with high ventricular rates often exceeding 200 beats per minute 1.

Diagnosis

  • Electrocardiographic Criteria: Incessant tachycardia with normal QRS morphology and atrioventricular dissociation 1.
  • Clinical Presentation: Elevated ventricular rate (140-370 beats/min), often associated with cardiac failure 1.
  • Recommended Tests: Electrocardiogram (ECG) is essential for diagnosis; echocardiography may help assess cardiac function and structural abnormalities 1.
  • Management

  • First-Line Treatments:
  • - Digoxin: No significant effect observed in the study 1.
  • Adjunctive Treatments:
  • - Propranolol: Limited success; partial response in some cases 1. - Amiodarone: Higher success rate with eight successes and three partial successes noted 1. - Combination Therapies: Use of other conventional agents showed limited efficacy with only two successes 1.

    Special Populations

  • Pediatrics: JET predominantly affects infants and young children, with significant morbidity related to cardiac failure 1.
  • Safety Concerns: Sudden deaths occurred with propranolol, amiodarone, and their combination, highlighting the need for careful monitoring 1.
  • Key Recommendations

  • Initiate Amiodarone for refractory cases: Consider amiodarone as a first-line adjunctive therapy for patients not responding to initial treatments due to its higher success rate 1 (Evidence: Moderate).
  • Avoid Sole Use of Digoxin: Do not rely solely on digoxin for managing JET due to its lack of significant effect 1 (Evidence: Strong).
  • Monitor for Sudden Cardiac Events: Closely monitor patients, especially those on propranolol or amiodarone, for potential sudden cardiac events or AV block 1 (Evidence: Weak).
