Intravascular large B-cell lymphoma

Last edited: 4/14/2026

Overview

Intravascular large B-cell lymphoma (IVL) is a rare and aggressive form of non-Hodgkin lymphoma characterized by the proliferation of neoplastic lymphoid cells within small blood vessel lumina, leading to multi-organ dysfunction and often rapid clinical deterioration 10111315.

Diagnosis

  • Clinical Presentation: Often presents with nonspecific symptoms including fever, neurological deficits, skin lesions, and multi-organ involvement 1011131415.
  • Imaging and Biopsy: Essential for diagnosis; biopsy showing neoplastic cells within vessel lumina is definitive 1017.
  • Immunophenotyping: Immunohistochemical staining for leukocyte common antigen (LCA) and cytokeratin can aid in distinguishing from other malignancies 17.
  • Differential Diagnosis: Requires exclusion of other causes like sepsis, especially in cases mimicking systemic inflammatory response syndrome 16.
  • Management

  • First-Line Treatment: Limited specific guidelines; often involves systemic chemotherapy regimens tailored to patient status 13.
  • Second-Line and Beyond: Chimeric antigen receptor (CAR) T-cell therapy recommended for early relapse (within 12 months) 3.
  • CAR-T Therapy Considerations: Minimize vein-to-vein time (V2Vt) to optimize outcomes 2.
  • Other Therapies: Platinum-based chemotherapy followed by high-dose chemotherapy with autologous stem cell rescue for late relapse 3.
  • Emerging Therapies: CD3xCD20 bispecific antibodies and other targeted therapies under investigation 35.
  • Special Populations

  • Elderly Patients: Management strategies may need adjustment due to higher comorbidities and frailty 13.
  • Comorbidities: Careful consideration of organ function and potential interactions with aggressive treatments 1015.
  • Key Recommendations

  • CAR-T Therapy for Early Relapse: Use CAR T-cell therapy in patients with relapsed/refractory large B-cell lymphoma within 12 months of first-line therapy (Evidence: Moderate) 3.
  • Minimize CAR-T Infusion Delays: Reduce vein-to-vein time to improve clinical outcomes in CAR T-cell therapy (Evidence: Moderate) 2.
  • Multidisciplinary Approach: Employ a comprehensive multidisciplinary strategy for diagnosis and management, emphasizing early biopsy and immunophenotyping (Evidence: Expert opinion) 1017.
