Immunoglobulin G4 related ophthalmic disease

Overview

Immunoglobulin G4 (IgG4) related ophthalmic disease involves inflammation mediated by IgG4 antibodies, commonly affecting structures such as the lacrimal gland (dacryoadenitis), extraocular muscles, and the optic nerve, often mimicking other inflammatory or neoplastic conditions. 3363

Diagnosis

  • Presence of elevated serum IgG4 levels or tissue IgG4+ plasma cells.
  • Characteristic imaging findings, such as enlargement of the lacrimal gland or extraocular muscles on MRI or CT.
  • Histopathological confirmation showing dense lymphoplasmocytic infiltration with abundant IgG4+ plasma cells.
