Unexplained chronic cough

Last edited: 4/14/2026

Overview

Chronic cough lasting more than 8 weeks is a common condition affecting approximately 10% of adults, often refractory to treatment of underlying causes and characterized by cough hypersensitivity 11321.

Diagnosis

  • Key Criteria: Cough duration >8 weeks 1613.
  • Recommended Tests:
  • - Comprehensive history and physical examination 113. - Pulmonary function tests 113. - Multidisciplinary investigations including ENT, GI, and pulmonology evaluations 1223. - Triple endoscopy in pediatric patients 12. - Consider GERD assessment with reflux finding score (RFS) 16.
  • Differential Diagnosis: Exclude common causes like asthma, GERD, and postnasal drip; consider less common etiologies 11324.
  • Management

  • First-Line Treatments:
  • - Address identifiable causes (e.g., asthma, GERD) with appropriate medications 11324. - Lifestyle modifications and smoking cessation 113.
  • Adjunctive Treatments:
  • - Speech-Language Pathology Therapy: Effective for reducing cough frequency and improving quality of life 47142634. - Behavioral Speech Therapy (BST): Demonstrates significant benefit in refractory cases 714. - Neuromodulators: Increasingly used by laryngologists for refractory cases 2. - Physiotherapy and Speech Therapy Interventions: Emerging non-pharmacological options showing promise 1719.

    Special Populations

  • Pediatrics: Chronic cough defined as >4 weeks in children aged 14 years or older; consider aerodigestive causes and multidisciplinary approaches 5912.
  • Elderly: Increased complexity due to comorbidities; thorough evaluation essential 13.
  • Comorbidities: Management should address coexisting conditions like GERD, asthma, and upper airway disorders 11316.
  • Key Recommendations

  • Exclude identifiable causes through comprehensive evaluation before considering unexplained chronic cough 1628 (Evidence: Strong).
  • Implement speech-language pathology therapy as a first-line adjunctive treatment for refractory chronic cough 47142634 (Evidence: Strong).
  • Consider multidisciplinary approaches including ENT, GI, and pulmonology evaluations for persistent cases 1223 (Evidence: Moderate).
  • Evaluate and manage GERD as part of the diagnostic workup due to its significant overlap with chronic cough 16 (Evidence: Moderate).
  • Use behavioral speech therapy for patients with refractory chronic cough, given its demonstrated efficacy 714 (Evidence: Strong).
