Lemierre syndrome

Last edited: 4/14/2026

Overview

Lemierre syndrome is a rare but life-threatening condition characterized by acute oropharyngeal infection, typically caused by Fusobacterium necrophorum, leading to septic thrombophlebitis of the internal jugular vein and metastatic septic emboli 1516.

Diagnosis

  • Clinical Presentation: Sore throat, fever, and signs of systemic infection 1516.
  • Key Diagnostic Criteria: Internal jugular vein thrombosis, oropharyngeal infection, and metastatic septic emboli 25.
  • Laboratory Tests: Elevated inflammatory markers, blood cultures often positive for Fusobacterium necrophorum 15.
  • Imaging: CT or MRI showing thrombophlebitis and abscesses 49.
  • Thrombosis Confirmation: Doppler ultrasound or venography for internal jugular vein thrombosis 34.
  • Management

  • Antibiotics: Initial broad-spectrum coverage followed by targeted therapy against Fusobacterium necrophorum (e.g., metronidazole, clindamycin) 1516.
  • Anticoagulation: Use of parenteral anticoagulation initially, transitioning to oral anticoagulants as appropriate 3.
  • Surgical Intervention: Drainage of abscesses and removal of infected material 410.
  • Supportive Care: Management of sepsis, respiratory support, and monitoring for complications 89.
  • Special Populations

  • Pediatrics: Lemierre syndrome can occur in children, requiring prompt recognition and aggressive management 614.
  • Comorbidities: Increased vigilance in patients with underlying conditions that may complicate diagnosis or management 11.
  • Key Recommendations

  • Early Recognition and Prompt Treatment: Initiate broad-spectrum antibiotics immediately upon suspicion, targeting Fusobacterium necrophorum 15 (Evidence: Strong).
  • Surgical Drainage for Abscesses: Drain abscesses surgically to prevent further metastatic spread 410 (Evidence: Moderate).
  • Anticoagulation Strategies: Employ appropriate anticoagulation to manage venous thrombosis, transitioning from parenteral to oral agents as clinically indicated 3 (Evidence: Moderate).
  • Respiratory Precautions: Implement respiratory precautions for healthcare workers managing suspected cases to prevent nosocomial transmission 7 (Evidence: Expert opinion).
