Ocular lateropulsion

Overview

Ocular lateropulsion refers to the involuntary movement of the eye towards one side, often associated with damage to the cranial nerves or structures within the orbit, leading to misalignment and potential diplopia. 3435

Diagnosis

  • Clinical Presentation: Diplopia, particularly when looking in a specific direction.
  • Neurological Examination: Assess cranial nerve function, particularly the abducens nerve (CN VI).
  • Imaging Studies: MRI or CT scans to evaluate for structural abnormalities or trauma in the orbit.
  • Ocular Motility Testing: Utilize prism bars or synoptophore to quantify the degree of lateropulsion. 3435

  • Conservative Management: Prism glasses to compensate for misalignment.
  • Surgical Intervention: For structural causes like orbital fractures or muscle entrapment, surgical realignment or decompression may be necessary.
  • Neuromodulation: In cases involving nerve palsies, consider rehabilitation techniques or botulinum toxin injections to relax overactive muscles. 3435
