Advanced cirrhosis

Last edited: 4/17/2026

Overview

Advanced cirrhosis represents a severe stage of liver disease characterized by significant liver dysfunction, portal hypertension, and increased risk of complications such as variceal bleeding, ascites, encephalopathy, and hepatorenal syndrome. Effective management focuses on symptom control, prevention of complications, and addressing quality of life.

Diagnosis

  • Clinical features: Jaundice, ascites, spider angiomas, palmar erythema, and signs of portal hypertension.
  • Laboratory tests: Elevated INR, low albumin, elevated bilirubin, and prolonged prothrombin time.
  • Imaging: Ultrasound or CT scan to assess liver morphology and portal vein patency.
  • Diagnostic procedures: Paracentesis for ascites analysis, endoscopy for varices detection 1234.
  • Management

  • Symptom Control:
  • - Pain management: Opioids (e.g., morphine) titrated to effect; consider non-opioid adjuncts like gabapentin 56. - Ascites: Diuretics (e.g., spironolactone, furosemide) to manage fluid retention 7. - Hepatic encephalopathy: Lactulose, rifaximin for ammonia reduction 8.
  • Complication Prevention:
  • - Variceal bleeding prophylaxis: Non-selective beta-blockers (e.g., propranolol) 9. - Infection prevention: Regular monitoring and prompt antibiotic therapy for suspected infections 10.
  • Deprescribing: Evaluate and taper unnecessary medications to reduce polypharmacy risks 222930.
  • Special Populations

  • Elderly: Increased risk of polypharmacy and adverse drug events; prioritize deprescribing and symptom-focused care 2230.
  • Comorbidities: Manage comorbidities like cardiovascular disease cautiously, considering drug interactions and organ function 2410.
  • Key Recommendations

  • Prioritize comprehensive symptom management, including opioid therapy for pain and non-pharmacological approaches for distress 56.
  • Implement deprescribing strategies to minimize medication burden and reduce adverse events in patients with limited life expectancy 2229.
  • Early integration of palliative care to address psychological, social, and spiritual needs alongside medical management 11931.
  • Regularly assess and manage complications such as ascites and variceal bleeding to improve quality of life 79.
  • Engage in goals-of-care discussions early to align treatment with patient preferences and values 1151.
  • (Evidence: Moderate)
