Fetal subdural intracranial hemorrhage

Last edited: 4/22/2026

Overview

Fetal subdural intracranial hemorrhage (SDH) is a rare antenatal complication characterized by bleeding into the subdural space of the fetal brain, often associated with increased perinatal morbidity and mortality, and potential neurodevelopmental delays 12.

Diagnosis

  • Key Diagnostic Criteria: Identified via prenatal ultrasound (USG) or magnetic resonance imaging (MRI) 12.
  • Recommended Tests: Routine third-trimester USG, with MRI for detailed imaging when necessary 12.
  • Grading: Cases often categorized into grades (I-IV) based on severity and extent of hemorrhage 2.
  • Management

  • First-Line Treatments: Stopping aspirin if implicated 1.
  • Adjunctive Treatments: Cesarean delivery may be indicated for severe cases; intra-amniotic injections like ethacridine lactate considered in specific scenarios 2.
  • Monitoring: Serial ultrasounds to monitor hematoma size reduction 1.
  • Special Populations

  • Pregnancy: Antenatal low-dose aspirin use may predispose to fetal SDH; discontinuation may mitigate risk 13.
  • Pediatrics: Follow-up until at least 18 months for neurodevelopmental assessment 1.
  • Key Recommendations

  • Discontinue low-dose aspirin if fetal SDH is suspected or diagnosed to potentially reduce hemorrhagic risk (Evidence: Moderate 13).
  • Utilize serial prenatal imaging (USG/MRI) for monitoring hematoma progression and response to interventions (Evidence: Moderate 12).
  • Consider cesarean delivery for severe cases (grades III-IV) to ensure prompt neonatal care (Evidence: Weak 2).
  • Initiate thorough neurodevelopmental follow-up in surviving infants to assess long-term outcomes (Evidence: Expert opinion 1).
