Lumbar radiculopathy

Last edited: 4/14/2026

Overview

Lumbar radiculopathy involves nerve root compression typically due to disc herniation or degenerative changes, leading to lower extremity pain, weakness, and sensory disturbances. 15

Diagnosis

  • Clinical Presentation: Pain radiating along a specific dermatome, neurological deficits correlating with nerve root distribution. 16
  • Physical Examination: Straight Leg Raise (SLR) test often positive; reliability varies. 4
  • Imaging: MRI is gold standard for visualizing disc herniations and nerve root compression. 12
  • Diagnostic Imaging Position: MRI with straight vs. flexed hips/knees shows differences in disc and nerve root findings; straight position may offer better visualization. 12
  • Transitional Vertebrae: Presence can complicate accurate diagnosis and treatment level selection. 2
  • Management

  • First-Line Treatments:
  • - Mechanical Traction: Effective for pain relief and functional improvement; prone traction is an alternative approach. 15 - Epidural Steroid Injections (ESIs): Used for short-term pain relief; cost-effectiveness debated. 1011 - Physical Therapy: Core strengthening, flexibility exercises, and modalities.
  • Adjunctive Treatments:
  • - Ultramicronized Palmitoylethanolamide (umPEA): Reported efficacy in nonsurgical cases. 14 - Radiofrequency Lesioning: For chronic pain refractory to other treatments; safety and efficacy in repeat procedures noted. 21 - Percutaneous Disc Decompression: Alternative to open discectomy for contained herniations. 18

    Special Populations

  • Pregnancy: Specific management considerations not detailed in abstracts.
  • Elderly: Long-term outcomes post-microdiscectomy show variable recovery; baseline characteristics poorly predict outcomes. 7
  • Comorbidities: IL-6 levels may influence recovery in patients with lumbar radicular pain. 20
  • Key Recommendations

  • Consider Mechanical Traction for conservative management of lumbar radiculopathy, including prone traction as an alternative approach. (Evidence: Moderate 15)
  • Use MRI with Straightened Position for better visualization of disc and nerve root abnormalities. (Evidence: Moderate 12)
  • Evaluate IL-6 Levels preoperatively to predict long-term recovery in patients with lumbar radicular pain. (Evidence: Moderate 20)
  • Delay Surgery beyond a certain period unless neurological deficits are present; optimal timing for surgery remains critical but not definitively quantified in the provided studies. (Evidence: Weak 9)
  • Consider Epidural Steroid Injections for short-term pain relief, weighing cost-effectiveness and patient-specific factors. (Evidence: Moderate 1011)
