Complex regional pain syndrome

Overview

Complex regional pain syndrome (CRPS) is a chronic pain condition characterized by severe pain, sensory abnormalities, and motor dysfunction, often following trauma or surgery, disproportionately affecting the extremities. 23

Diagnosis

  • Clinical Criteria: Based on signs and symptoms including pain disproportionate to injury, sensory abnormalities, edema, skin color changes, and motor impairments. 2314
  • Recommended Tests: Quantitative sensory testing (QST) can help characterize sensory phenotypes but is not diagnostic on its own. 4
  • Grading: No universally accepted grading system; clinical assessment guides severity. 7
  • Management

  • First-Line Treatments:
  • - Physical Therapy: Including pain exposure physical therapy to address motor control and fear avoidance. 183 - Sympathetic Blocks: Such as guanethidine for early-phase CRPS to reduce sympathetic overactivity. 21
  • Adjunctive Treatments:
  • - Pharmacotherapy: Antidepressants (e.g., duloxetine), anticonvulsants (e.g., gabapentin), and NMDA receptor antagonists (e.g., ketamine). 193 - Spinal Cord Stimulation: For refractory cases, particularly with paddle leads under conscious sedation to minimize complications. 15 - Remote Ischemic Conditioning (RIC): Emerging evidence suggests potential benefits in improving tissue perfusion. 13
