Eye infection

Last edited: 4/13/2026

Overview

Eye infections encompass a range of conditions affecting ocular structures, often requiring prompt clinical assessment and management to prevent complications such as vision loss. These infections can arise from various pathogens and are encountered across different demographics, with significant implications for public health, especially in resource-limited settings. 13184150

Diagnosis

  • Clinical Presentation: Redness, discharge, pain, photophobia, and visual disturbances.
  • Physical Examination: Slit-lamp examination to identify conjunctival injection, corneal ulcers, or other signs.
  • Laboratory Tests: Microbiological cultures from conjunctival swabs or corneal scrapings to identify pathogens. 616274150
  • Imaging: Rarely needed but may include fluorescein staining to assess corneal integrity. 627
  • Management

  • Antibiotic Therapy: Topical antibiotics (e.g., fluoroquinolones, aminoglycosides) based on culture sensitivity results. 616274150
  • Antiviral Agents: For viral conjunctivitis, consider antiviral drops (e.g., trifluridine). 616274150
  • Supportive Care: Lubricants, cold compresses, and ensuring proper hygiene.
  • Surgical Intervention: For severe cases like endophthalmitis or perforating injuries, surgical drainage or repair may be necessary. 616274150
  • Special Populations

  • Pediatrics: Increased vigilance for signs of abuse in non-accidental injuries; consider prophylactic topical antibiotics post-trauma. 1141
  • Elderly: Higher risk of complications; thorough evaluation for underlying comorbidities affecting healing. 3541
  • Comorbidities: Patients with diabetes or immunodeficiency require closer monitoring and tailored antibiotic therapy. 1641
  • Key Recommendations

  • Prompt Referral and Assessment: Urgent referral to ophthalmology for suspected severe ocular infections or injuries to prevent vision loss. (Evidence: Strong 1318)
  • Culturing Before Antibiotic Selection: Always perform microbiological cultures before initiating broad-spectrum antibiotic therapy to guide targeted treatment. (Evidence: Moderate 61627)
  • Protective Eyewear: Mandatory use of appropriate protective eyewear in high-risk occupational and recreational settings to prevent eye injuries. (Evidence: Expert opinion 52449)
  • Enhanced Training for Emergency Staff: Continuous training for emergency department staff in managing ocular emergencies to improve outcomes. (Evidence: Moderate 61356)
  • Preventive Measures in High-Risk Groups: Implement targeted preventive strategies in vulnerable populations such as elderly and children to reduce incidence of eye injuries and infections. (Evidence: Moderate 31135)
