Overview
Chronic periaortitis, also referred to as idiopathic retroperitoneal fibrosis, encompasses inflammatory conditions affecting the periaortic region, potentially involving the aorta and adjacent structures. It is hypothesized to arise from an autoallergic reaction to components of atherosclerotic plaques 3.Diagnosis
Elevated levels of anti-oxidized low-density lipoprotein (OxLDL) antibodies, particularly IgM, may be observed 1.
Histopathological confirmation showing chronic inflammatory changes around the aorta is definitive 3.
Imaging studies (CT, MRI) can reveal inflammatory masses or aneurysms adjacent to the aorta 23.
Clinical presentation may include symptoms related to compression of adjacent structures, such as biliary obstruction 2.Management
First-line: Corticosteroids are highly effective and recommended as initial treatment 3.
Adjunctive: Surgical ureterolysis may be necessary in cases of ureteric obstruction 3.
Monitoring: Lipid profile and atherosclerotic biomarkers should be monitored, especially in patients with cardiovascular disease 1.Special Populations
Comorbidities: Patients with cardiovascular disease may exhibit different antibody profiles (e.g., lower aOxLDL-IgM titers) 1.
Gender Differences: Female patients might have higher levels of aOxLDL-IgM antibodies compared to males 1.Key Recommendations
Initiate corticosteroid therapy as first-line treatment for chronic periaortitis (Evidence: Strong 3).
Consider histopathological examination for definitive diagnosis 3.
Monitor lipid profiles and atherosclerotic biomarkers, particularly in patients with cardiovascular involvement (Evidence: Moderate 1).References
