Orbital lymphoma

Last edited: 4/13/2026

Overview

Orbital lymphoma refers to malignant lymphoid neoplasms affecting the orbit, often presenting as a mass with potential for infiltration into surrounding structures, causing symptoms such as proptosis, pain, and visual disturbances. 73747

Diagnosis

  • Clinical Presentation: Proptosis, pain, diplopia, and visual impairment.
  • Imaging: CT and MRI are essential for characterizing the extent and nature of the lesion. Diffusion-weighted imaging (DWI) can provide additional insights into lesion characteristics. 729
  • Histopathology: Biopsy is crucial for definitive diagnosis, distinguishing between reactive conditions and lymphoma. 47
  • Laboratory Tests: Elevated inflammatory markers may be seen but are non-specific; comprehensive blood work can help rule out systemic involvement. 34
  • Management

  • Primary Treatment: Radiation therapy is a common first-line treatment, often combined with systemic chemotherapy for advanced cases. 47
  • Systemic Therapy: Chemotherapy regimens such as CHOP (Cyclophosphamide, Doxorubicin, Vincristine, Prednisone) may be used, especially for advanced or recurrent disease. 47
  • Surgical Intervention: Reserved for complications like orbital decompression for severe proptosis or biopsy when imaging is inconclusive. 3150
  • Monitoring: Regular follow-up with imaging and clinical assessments to monitor response and detect recurrence. 537
  • Special Populations

  • Pediatrics: Rare but requires multidisciplinary management due to unique anatomical considerations and potential for aggressive behavior. 1219
  • Elderly: Increased risk of comorbidities affecting treatment tolerance and outcomes; tailored management plans are essential. 35
  • Comorbidities: Patients with systemic lymphoma require coordinated care addressing both local and systemic disease. 47
  • Key Recommendations

  • Definitive Diagnosis Requires Histopathological Confirmation: Biopsy is essential for distinguishing orbital lymphoma from other inflammatory or neoplastic conditions. (Evidence: Moderate 47)
  • Radiation Therapy as Primary Treatment: Effective for localized disease, often used as initial management. (Evidence: Moderate 47)
  • Systemic Chemotherapy for Advanced Disease: Consider CHOP regimen for patients with advanced or recurrent orbital lymphoma. (Evidence: Moderate 47)
  • Regular Follow-Up with Imaging and Clinical Assessments: Crucial for monitoring response and detecting recurrence. (Evidence: Expert opinion 5)
