Basal cell carcinoma

Last edited: 4/13/2026

Overview

Basal cell carcinoma (BCC) is the most common type of skin cancer, predominantly affecting fair-skinned individuals. It is characterized by slow growth and low metastatic potential but can cause significant local tissue destruction if left untreated 167.

Diagnosis

  • Clinical examination often sufficient for diagnosis, especially in typical presentations 117.
  • Histopathological confirmation recommended for atypical or clinically uncertain lesions 117.
  • Optical coherence tomography (OCT) shows promise in differentiating BCC from non-BCC lesions 4.
  • Fluorescence confocal mosaicking microscopy (CMM) aids in precise intraoperative assessment 22.
  • AI tools are being developed for accurate diagnosis but require reliable reference standards 3.
  • Management

  • Primary Treatment:
  • - Surgical excision (Mohs micrographic surgery recommended for high-risk sites) 1172131. - Curettage and electrodesiccation for superficial BCCs 117.
  • Advanced/Locally Advanced BCC:
  • - Hedgehog pathway inhibitors (e.g., vismodegib, sonidegib) for systemic therapy 91223. - Monitor for adverse effects, particularly squamous cell carcinoma development 25.
  • Adjuvant Measures:
  • - Regular follow-up essential due to high recurrence risk 1847. - Additional Mohs slides recommended to optimize surgical outcomes 31.

    Special Populations

  • Pregnancy: Limited data; conservative management preferred due to potential teratogenic risks of systemic therapies 12.
  • Elderly: Increased risk of recurrence and complications; tailored follow-up and management strategies advised 18.
  • Comorbidities: Consider impact on treatment tolerance and efficacy; individualized care plans necessary 18.
  • Key Recommendations

  • Surgical excision, particularly Mohs micrographic surgery, is recommended for high-risk BCC sites to minimize recurrence rates (Evidence: Strong 11721).
  • Systemic hedgehog pathway inhibitors should be considered for locally advanced BCC, with close monitoring for adverse effects (Evidence: Moderate 91223).
  • Regular follow-up is crucial for all BCC patients due to the risk of recurrence and development of new lesions (Evidence: Strong 1847).
  • Use of advanced imaging techniques like OCT and CMM can enhance diagnostic accuracy and surgical precision (Evidence: Moderate 422).
  • AI tools should be validated with reliable reference standards before clinical implementation for BCC diagnosis (Evidence: Expert opinion 3).
