Peritonsillar abscess

Overview

Peritonsillar abscess (PTA) is a deep neck infection characterized by pus accumulation in the peritonsillar space, often complicating acute tonsillitis and potentially leading to severe complications such as airway obstruction if untreated 1222.

Diagnosis

  • Clinical Presentation: Fever, sore throat, dysphagia, trismus, and muffled voice (hot potato voice) 12666.
  • Diagnostic Imaging: Ultrasound is often first-line, with computed tomography (CT) reserved for complex cases or vascular assessment 353035.
  • Ultrasound Findings: Presence of fluid collection in the peritonsillar space, hypoechogenic with posterior enhancement 3535.
  • CT Findings: Useful for assessing complications like parapharyngeal space involvement and vascular proximity 530.
  • Cytobacteriological Testing: Not routinely contributive in clinical practice 18.
  • Management

  • First-Line Treatment: Needle aspiration or incision and drainage under local anesthesia 132266.
  • Antibiotics: Penicillin or amoxicillin as first-line, with metronidazole or clindamycin considered for anaerobic coverage 82838.
  • Conservative Management: Antibiotics alone may be considered in selected pediatric cases, but drainage is often necessary 111233.
  • Surgical Intervention: Tonsillectomy may be performed immediately or deferred based on recurrence risk and patient factors 132748.
