Tuberculoma

Overview

Tuberculomas are focal granulomatous lesions caused by Mycobacterium tuberculosis, typically occurring within the brain parenchyma, but can also affect other regions such as the spine, cavernous sinus, and intrasellar areas 1234691011.

Diagnosis

  • Imaging Characteristics: Ring-enhancing lesions on CT or MRI are common; larger lesions often show ring enhancement, while smaller ones may have homogeneous enhancement 5.
  • Histopathological Confirmation: Essential for definitive diagnosis, often revealing acid-fast bacilli 7.
  • Clinical Presentation: Variable, including mass effect symptoms, seizures, and atypical presentations like optic perineuritis 10.
  • Extracranial Involvement: May coexist with miliary tuberculosis or other extrapulmonary manifestations 7.
  • Management

  • Antituberculous Therapy: Standard regimen includes isoniazid (e.g., 300 mg daily), rifampin, ethambutol, and pyrazinamide for initial phase, followed by continuation phase with isoniazid and rifampin 6.
  • Surgical Intervention: Indicated for symptomatic lesions causing significant mass effect, neurological deficits, or when imaging is inconclusive 71011.
  • Monitoring: Serial imaging (CT/MRI) to assess response to treatment and regression of lesions 5.
  • Special Populations

  • Pediatrics: Specific case reports indicate successful surgical intervention followed by antituberculous treatment 9.
  • Elderly: A case report highlights successful surgical removal and recovery in an elderly patient 3.
  • Comorbidities: No specific guidelines provided; management tailored to individual clinical presentation and imaging findings 78.
  • Key Recommendations

  • Histopathological Confirmation is Essential for Diagnosis (Evidence: Moderate 7).
  • Combined Medical and Surgical Treatment Should Be Considered for Symptomatic Lesions (Evidence: Weak 71011).
  • Serial Imaging is Recommended to Monitor Response to Antituberculous Therapy (Evidence: Moderate 5).
