Torsion of right ovary and fallopian tube

Last edited:

Epidemiology

The paper underscores the rarity of isolated fallopian tube torsion, especially noting its uncommon occurrence during pregnancy [PMID:21608440].

From 1936 to the publication date, only 19 cases of isolated fallopian tube torsion in pregnancy treated surgically were retrieved [PMID:19493607].

Clinical Presentation

This case report describes a patient with IFTT in early pregnancy who presented with symptoms mimicking an appendicular lump, initially considered for medical termination of pregnancy [PMID:21608440].

All reviewed cases presented with lower quadrant abdominal pain, predominantly on the right side (90%) [PMID:19493607].

A case report [PMID:6519345] highlights torsion of the pregnant fallopian tube as a rare but important differential diagnosis for abdominal pain in pregnant women.

Diagnosis

Ultrasound detected an adnexal cyst in all surgically evaluated cases where imaging was performed (8 patients), and Doppler flow ultrasound was highlighted as a novel diagnostic tool for precise differentiation [PMID:19493607].

Differential Diagnosis

The authors highlight that IFTT is frequently misdiagnosed as acute appendicitis or ovarian torsion, emphasizing the importance of considering these conditions in pregnant patients presenting with abdominal pain [PMID:21608440].

Despite its rarity, torsion of the pregnant fallopian tube must be included in the differential diagnosis for abdominal pain in pregnant females [PMID:6519345].

Management

The authors recommend early surgical intervention using microsurgical techniques to salvage the affected tube [PMID:6519345].
