Ophthalmoplegia

Last edited: 4/14/2026

Overview

Ophthalmoplegia refers to the paralysis or weakness of one or more extraocular muscles, leading to impaired eye movement and often associated with systemic conditions such as abetalipoproteinemia. 61518

Diagnosis

  • Clinical Presentation: Characteristic eye movement disorders including diplopia, blepharoptosis, and extraocular muscle weakness. 918
  • Ophthalmologic Examination: Detailed fundoscopy and slit-lamp examination to assess retinal changes and muscle function. 18
  • Electrophysiological Tests: Somatosensory evoked potentials (SEPs) may show abnormalities correlating with neuropathology. 14
  • Laboratory Tests: Low lipid profile, presence of acanthocytes in peripheral blood, and vitamin E deficiency assessment. 5815
  • Genetic Testing: Consideration for genetic confirmation in suspected abetalipoproteinemia cases. 513
  • Management

  • Vitamin Supplementation: High-dose vitamin E and other fat-soluble vitamins (A, D, K) to address deficiencies. 51518
  • Dietary Modifications: Medium-chain triglyceride (MCT) formulas to manage fat malabsorption. 8
  • Symptomatic Treatment: Addressing associated symptoms like diplopia with prism glasses or surgical interventions if necessary. 9
  • Monitoring: Regular follow-up with electrophysiological studies to assess disease progression and treatment efficacy. 14
  • Special Populations

  • Pediatrics: Early diagnosis and intervention with dietary modifications and vitamin supplementation are crucial. 815
  • Elderly: Increased vigilance for neurological complications and regular monitoring of vitamin levels. 1415
  • Comorbidities: Patients with abetalipoproteinemia may require additional management for retinal degeneration and ataxia. 1518
  • Key Recommendations

  • Initiate high-dose vitamin E supplementation for patients diagnosed with abetalipoproteinemia to manage ophthalmoplegia and neurological symptoms. (Evidence: Strong 515)
  • Implement dietary modifications, particularly MCT formulas, to alleviate fat malabsorption and associated ophthalmoplegic symptoms. (Evidence: Moderate 8)
  • Regularly monitor patients with abetalipoproteinemia using electrophysiological tests like SEPs to track disease progression and treatment response. (Evidence: Moderate 14)
