Portal triaditis

Last edited: 4/14/2026

Overview

Portal triaditis refers to inflammation affecting the structures within the portal triad, including the bile ducts, hepatic artery, and portal vein, often seen in the context of non-cirrhotic portal hypertension 12.

Diagnosis

  • Key Diagnostic Criteria: Presence of portal vein thrombosis, ectopic varices, and non-cirrhotic portal hypertension 1.
  • Recommended Tests: Doppler ultrasonography to assess portal vein thrombosis and varices 1.
  • Clinical Signs: Hypotension, hematemesis, and melena may indicate active bleeding 1.
  • Unique Findings: Cruveilhier-Baumgarten venous hum, indicative of portal vein hypertension, may be observed 4.
  • Management

  • First-Line Treatments: Cyanoacrylate injection for managing ectopic varices 1.
  • Adjunctive Treatments: Management of underlying causes such as portal vein thrombosis 1.
  • Monitoring: Long-term follow-up to assess for complications 1.
  • Special Populations

  • Comorbidities: Hypoxaemia and digital clubbing can occur in non-cirrhotic portal hypertension, requiring pulmonary function assessment 2.
  • Key Recommendations

  • Cyanoacrylate injection is effective for managing ectopic varices in non-cirrhotic portal hypertension (Evidence: Strong 1).
  • Doppler ultrasonography should be utilized for diagnosing portal vein thrombosis and identifying ectopic varices 1.
  • Patients with non-cirrhotic portal hypertension should be monitored for hypoxaemia and intrapulmonary shunting, particularly if presenting with digital clubbing 2 (Evidence: Moderate).
  • The Cruveilhier-Baumgarten venous hum can be a diagnostic marker for portal vein hypertension, distinguishing it from normal variants 4 (Evidence: Expert opinion).
