Malignant neoplasm of round ligament

Last edited: 4/15/2026

Overview

Round ligament varicosities (RLVs) are a rare vascular condition predominantly observed during pregnancy, often mistaken for inguinal hernias due to similar clinical presentations. 12

Diagnosis

  • Clinical Presentation: Painless or painful inguinal swelling, often reducible. 2
  • Physical Examination: Non-specific findings; may mimic inguinal hernia. 1
  • Ultrasonography: Essential diagnostic tool; shows echo-free tubular structures with vascularity on color Doppler, characteristic of RLVs. 123
  • Valsalva Maneuver: Enhances visibility of RLVs on imaging. 1
  • Management

  • Conservative Therapy: Recommended initial approach, including reassurance and observation. 2
  • Postpartum Resolution: Symptoms typically resolve spontaneously after delivery. 12
  • Avoid Unnecessary Surgery: Prevents surgical intervention for benign condition. 2
  • Special Populations

  • Pregnancy: Common occurrence in second or third trimester; conservative management is safe and effective. 2
  • Postpartum: Symptoms usually resolve without intervention postpartum. 12
  • Key Recommendations

  • Utilize Ultrasonography for Diagnosis: Essential for differentiating RLVs from inguinal hernias in pregnant patients. (Evidence: Moderate) 23
  • Adopt Conservative Management: Initial treatment should focus on reassurance and observation, avoiding unnecessary surgical interventions. (Evidence: Moderate) 2
  • Expect Spontaneous Resolution Postpartum: Monitor patients expectantly, as symptoms typically resolve after delivery. (Evidence: Weak) 12
