Acute respiratory infections

Last edited: 4/14/2026

Overview

Acute respiratory infections (ARIs) encompass a broad range of illnesses affecting the respiratory tract, including upper respiratory infections (e.g., otitis media, sinusitis) and lower respiratory infections (e.g., pneumonia, bronchiolitis). Household air pollution and environmental factors significantly contribute to the burden of ARIs, particularly in vulnerable populations such as children under five 121.

Diagnosis

  • Clinical Presentation: Symptoms include cough, fever, sore throat, rhinorrhea, and difficulty breathing 3.
  • Physical Examination: Focus on respiratory signs such as wheezing, crackles, and decreased breath sounds 3.
  • Diagnostic Tests:
  • - Radiography: Chest X-rays for suspected pneumonia 20. - Nasopharyngeal Swabs: For viral detection (e.g., influenza, RSV) 17. - Laboratory Tests: Blood tests for leukocytosis, C-reactive protein levels 19.

    Management

  • Supportive Care: Hydration, rest, fever management 3.
  • Antibiotics: Reserved for confirmed bacterial infections (e.g., pneumonia, severe cases) 2728.
  • Vaccinations: Influenza and pneumococcal vaccines recommended to prevent severe infections 629.
  • Vitamin D: Consider supplementation in deficient children to potentially reduce infection risk 513.
  • Environmental Interventions: Reduce exposure to household air pollution and ETS 121.
  • Special Populations

  • Pediatrics: Higher susceptibility to ARIs; emphasize vaccinations and supportive care 313.
  • Elderly: Increased risk of complications; focus on preventive measures and early intervention 219.
  • Comorbidities: Patients with chronic respiratory conditions (e.g., asthma) require tailored management plans 715.
  • Key Recommendations

  • Implement Vaccination Programs: Administer influenza and pneumococcal vaccines to reduce severe respiratory infections in high-risk populations (Evidence: Strong 629).
  • Promote Environmental Health: Minimize exposure to household air pollution and environmental tobacco smoke to lower ARI incidence in children (Evidence: Moderate 121).
  • Judicious Antibiotic Use: Prescribe antibiotics only for confirmed bacterial infections to combat antibiotic resistance (Evidence: Moderate 2728).
  • Consider Vitamin D Supplementation: Evaluate and supplement vitamin D in deficient children to potentially reduce respiratory infection risk (Evidence: Moderate 513).
  • Enhance Supportive Care: Provide adequate hydration, fever management, and respiratory support for pediatric and elderly patients (Evidence: Expert opinion).
