Recurrent acute tonsillitis

Last edited: 4/14/2026

Overview

Recurrent acute tonsillitis (RAT) is characterized by multiple episodes of tonsillar inflammation typically caused by bacterial infections, most commonly Streptococcus pyogenes, leading to significant morbidity and often necessitating surgical intervention such as tonsillectomy. 1512

Diagnosis

  • Clinical Criteria: Multiple episodes of tonsillitis (typically ≥7 episodes in one year or ≥5 episodes over two consecutive years).
  • Laboratory Tests: Throat cultures may identify causative pathogens, often Streptococcus pyogenes.
  • Imaging/Pathology: Not routinely required but histopathological examination of tonsil tissue can reveal chronic inflammation or specific pathogens like actinomyces. 118
  • Management

  • First-Line Treatment: Antibiotics (e.g., penicillin or amoxicillin) for acute episodes. 5
  • Surgical Intervention: Tonsillectomy is often considered for severe or recurrent cases unresponsive to medical management. Techniques include cold steel dissection and laser dissection, with comparative efficacy and safety needing further evidence. 14
  • Postoperative Care: Standard perioperative management guidelines should be followed to minimize complications. 4
  • Special Populations

  • Pediatrics: Tonsillectomy is frequently indicated in children with recurrent tonsillitis, though referral criteria may vary between primary care physicians and otolaryngologists. 710
  • Comorbidities: Patients with underlying conditions like immunodeficiency may require tailored management approaches, considering potential altered immune responses. 5
  • Key Recommendations

  • Consider Tonsillectomy for Severe Recurrent Acute Tonsillitis: Indicated in patients with ≥7 episodes annually or ≥5 episodes over two years, unresponsive to medical treatment. (Evidence: Moderate 14)
  • Evaluate Surgical Techniques: While laser tonsillectomy is an alternative, comparative evidence with cold steel dissection is still evolving. (Evidence: Weak 1)
  • Follow Evidence-Based Perioperative Guidelines: To minimize surgical risks and optimize recovery in pediatric and adult patients. (Evidence: Moderate 4)
