Postprocedural subglottic stenosis

Last edited: 4/13/2026

Overview

Postprocedural subglottic stenosis refers to narrowing of the subglottic region following endoscopic or surgical interventions, often necessitating further endoscopic dilation or more invasive reconstructive surgery. 2851

Diagnosis

  • Clinical Presentation: Hoarseness, dyspnea, stridor, and reduced vocal fold mobility.
  • Diagnostic Imaging: Flexible laryngoscopy to visualize stenosis and assess severity.
  • Grading Systems: Categorized using the Cotton-Myer grading system (I-IV) based on the degree of subglottic narrowing 51.
  • Management

  • Initial Treatment: Endoscopic dilation with rigid or flexible dilators.
  • Adjunctive Therapies: Use of steroid injections to reduce inflammation 51.
  • Recurrent Stenosis: Consider surgical reconstruction, such as cricopharyngeal myotomy or augmentation procedures 28.
  • Postoperative Care: Regular follow-up laryngoscopy to monitor healing and recurrence 51.
  • Special Populations

  • Pediatrics: Careful monitoring and conservative management initially due to potential for spontaneous resolution 51.
  • Elderly: Consider comorbidities and functional status when planning interventions; endoscopic approaches favored to minimize risks 28.
  • Key Recommendations

  • Initial Endoscopic Dilation: Employ endoscopic dilation as the first-line treatment for postprocedural subglottic stenosis (Evidence: Moderate 51).
  • Steroid Injections for Inflammation: Utilize steroid injections to manage inflammation and facilitate dilation (Evidence: Moderate 51).
  • Surgical Reconstruction for Recurrence: Consider surgical interventions like augmentation for recurrent or severe stenosis (Evidence: Expert opinion 28).
  • Regular Follow-Up: Schedule frequent laryngoscopic evaluations to assess healing and detect recurrence early (Evidence: Expert opinion 51).
