Anxiety disorder

Last edited: 4/23/2026

Overview

Anxiety disorders encompass a range of conditions characterized by excessive fear, worry, or anxiety that interferes with daily functioning. These disorders include generalized anxiety disorder, social anxiety disorder, panic disorder, and specific phobias. 42443

Diagnosis

  • Key Diagnostic Criteria: Excessive anxiety and worry about multiple aspects of life occurring on more days than not for at least six months, difficult to control, and leading to significant distress or impairment. 443
  • Recommended Tests: No specific laboratory tests; diagnosis primarily based on clinical interviews and standardized assessment tools. 43
  • Grading: Severity often graded based on symptomatology and functional impairment using scales like the Hamilton Anxiety Rating Scale (HARS) or the Generalized Anxiety Disorder 7-item scale (GAD-7). 43
  • Management

  • First-Line Treatments:
  • - Psychotherapy: Cognitive Behavioral Therapy (CBT) is highly effective 61566. - Pharmacotherapy: Selective Serotonin Reuptake Inhibitors (SSRIs) and Serotonin-Norepinephrine Reuptake Inhibitors (SNRIs) are commonly prescribed 4566.
  • Adjunctive Treatments:
  • - Mindfulness-Based Interventions: Shown to be effective, particularly for social anxiety disorder 2236. - Beta-Blockers: Used for situational anxiety but not as primary treatment 5. - Cannabidiol (CBD): Emerging evidence suggests potential benefits 13. - Brain Stimulation: Such as Transcranial Magnetic Stimulation (TMS) and Electroconvulsive Therapy (ECT) for treatment-resistant cases 7.

    Special Populations

  • Pregnancy: Screening and management should consider potential risks and benefits of pharmacological treatments; CBT is recommended 2629.
  • Pediatrics: Early intervention with CBT is crucial; subclinical cardiovascular risk should be monitored 1643.
  • Elderly: Caution with benzodiazepines due to increased risk of adverse effects; SSRIs are preferred 1056.
  • Comorbidities: Attention to interactions with other medications; comorbid depression often requires integrated treatment approaches 8858.
  • Key Recommendations

  • Screen for Anxiety Disorders Regularly, especially in pediatric and adolescent populations, using validated tools 2643 (Evidence: Strong)
  • Primary Treatment with CBT for most anxiety disorders, supplemented by pharmacotherapy if necessary 615 (Evidence: Strong)
  • Avoid Long-Term Benzodiazepine Use in elderly patients due to risk of adverse effects; prefer SSRIs or SNRIs 1056 (Evidence: Moderate)
  • Monitor Cardiovascular Risk in young adults with chronic anxiety due to potential links with sleep irregularity 3 (Evidence: Moderate)
  • Screen for Anxiety in Pregnant Women and consider non-pharmacological interventions first 2629 (Evidence: Moderate)
