Angiomyolipoma

Last edited: 4/14/2026

Overview

Angiomyolipomas (AMLs) are benign mesenchymal tumors most commonly found in the kidney, often associated with tuberous sclerosis. They are typically asymptomatic but can present with complications such as hemorrhage or rupture 12.

Diagnosis

  • Imaging Characteristics: CT and MRI are crucial; presence of fat is key for diagnosis 2.
  • Fat Identification: Essential for distinguishing AML from other renal masses 2.
  • Histopathology: Confirms diagnosis, showing characteristic smooth muscle, fat, and blood vessel components 3.
  • Extrarenal AMLs: Rare but can occur; diagnosis similar but context important 7.
  • Management

  • Observation: Majority can be monitored without intervention if asymptomatic 2.
  • Embolization: First-line for preventing or managing hemorrhage; subselective particle embolization preferred 2.
  • Surgical Intervention: Reserved for cases with rupture, large size, or suspected malignancy 12.
  • Nephrectomy: Considered in malignant transformation or extensive involvement 3.
  • Special Populations

  • Tuberous Sclerosis: AMLs more frequent; careful monitoring advised 23.
  • Extrarenal AMLs: Treated surgically with simple excision; no reported cases with tuberous sclerosis 7.
  • Key Recommendations

  • Monitor asymptomatic AMLs without intervention unless risk factors for complications are present (Evidence: Moderate 2).
  • Use subselective particle embolization for managing or preventing hemorrhage in symptomatic or high-risk AMLs (Evidence: Moderate 2).
  • Consider surgical options such as nephrectomy for malignant transformation or extensive intrarenal extension (Evidence: Weak 3).
