Imperforate large intestine

Last edited: 4/15/2026

Overview

Imperforate hymen is a congenital anomaly characterized by the absence of an opening in the hymen, leading to obstruction of the vaginal canal and accumulation of menstrual blood, resulting in conditions such as hematocolpos and constipation 12.

Diagnosis

  • Clinical Presentation: Firm, tender suprapubic mass, difficulty voiding, chronic constipation, and lumbago 12.
  • Diagnostic Imaging:
  • - Emergency Point-of-Care Ultrasound: Useful for initial bedside diagnosis 1. - Comprehensive Abdominal Ultrasound: Confirms initial findings 1. - Magnetic Resonance Imaging (MRI): Provides detailed visualization for definitive diagnosis 1.
  • Differential Diagnosis: Consider in adolescents with unexplained low back pain and chronic constipation, especially if menstruation is denied 2.
  • Management

  • Medical Management:
  • - Oral Contraceptives: Used to reduce bleeding and manage symptoms preoperatively 1.
  • Surgical Management:
  • - Hymenectomy: Outpatient surgical intervention recommended following consultation with gynecology 1.

    Special Populations

  • Pediatrics: Common presentation in adolescent girls; careful consideration of chronic symptoms like constipation and back pain is crucial 12.
  • Key Recommendations

  • Utilize emergency point-of-care ultrasound for rapid diagnosis of hematocolpometra in pediatric patients with suprapubic masses 1 (Evidence: Moderate).
  • Consider imperforate hymen in the differential diagnosis of adolescent girls presenting with chronic constipation and unexplained low back pain 2 (Evidence: Moderate).
  • Schedule outpatient surgical hymenectomy following imaging confirmation and gynecology consultation 1 (Evidence: Expert opinion).
