Meconium ileus

Last edited: 4/15/2026

Overview

Meconium ileus is a condition characterized by the obstruction of the ileum by solidified meconium, typically seen in neonates with cystic fibrosis. It often presents as intestinal obstruction shortly after birth. 1

Diagnosis

  • Ultrasonographic findings: Progressive dilatation of the fetal intestines suggestive of obstruction.
  • Amniocentesis and amniography: Utilized to assess fetal condition and response to potential interventions.
  • Postnatal confirmation: Large amounts of watery meconium passed postnatally may indicate resolution of obstruction 1.
  • Management

  • In utero intervention: Amniography with Urografin may relieve fetal intestinal obstruction (observed in case reports).
  • Postnatal management: Surgical intervention (such as ileostomy or resection) is often required for definitive treatment.
  • Supportive care: Includes fluid resuscitation and monitoring for complications like perforation or sepsis.
  • Special Populations

  • Pregnancy: In utero treatment with amniography using Urografin shows potential benefits despite risks 1.
  • Key Recommendations

  • Consider in utero amniography with Urografin for suspected fetal meconium ileus to potentially relieve obstruction (Evidence: Weak) 1
  • Prompt postnatal surgical evaluation and intervention are crucial for managing meconium ileus (Evidence: Expert opinion)
  • Monitor for and manage complications such as perforation or sepsis post-intervention (Evidence: Expert opinion)
