Myelolipoma

Overview

Myelolipoma is a rare, benign neoplasm composed of mature adipose tissue and hematopoietic elements, typically found in the adrenal glands but also reported in the lungs, liver, and accessory adrenal glands 12345678.

Diagnosis

  • Clinical Presentation: Often asymptomatic, discovered incidentally as nodules on imaging 234.
  • Imaging: Characteristic findings on CT and ultrasound include fat-containing masses with heterogeneous density 567.
  • Pathology: Definitive diagnosis through histopathological examination with immunocytochemical staining 3.
  • Differential Diagnosis: Important to distinguish from other adrenal masses, including adenomas and lipomas 5.
  • Management

  • Surgical Intervention: Indicated for symptomatic lesions or when diagnosis is uncertain to exclude malignancy 45.
  • Observation: Asymptomatic lesions may be managed conservatively with regular imaging follow-up 23.
  • No Specific Medical Therapy: No pharmacological treatment is typically required 4.
  • Special Populations

  • Pediatrics: Can occur in children, as seen in a case with homozygous beta thalassemia 4.
  • Comorbidities: No specific management adjustments noted for comorbidities; management remains focused on the lesion itself 4.
  • Key Recommendations

  • Imaging for Diagnosis: Utilize CT and ultrasound for initial characterization of suspected myelolipomas (Evidence: Moderate 567).
  • Surgical Consultation for Symptomatic Lesions: Consider surgical excision for symptomatic or large myelolipomas to rule out malignancy (Evidence: Moderate 4).
  • Conservative Management for Asymptomatic Lesions: Asymptomatic lesions can be managed with surveillance imaging to monitor for changes (Evidence: Expert opinion 23).
