Wernicke's disease

Last edited: 4/14/2026

Overview

Wernicke's encephalopathy (WE) is an acute neurological disorder caused by severe thiamine (vitamin B1) deficiency, often associated with chronic alcoholism but also seen in conditions like malnutrition, bariatric surgery, pregnancy complications, and prolonged parenteral nutrition 1235101319.

Diagnosis

  • Key Symptoms: Altered mental status, ophthalmoplegia (nystagmus, lateral rectus palsy), ataxia 12371119.
  • Recommended Tests: Brain MRI showing characteristic changes in the mammillary bodies, periaqueductal region, and splenium of the corpus callosum; thiamine levels may be supportive but not universally available 2711.
  • Clinical Grading: No standardized grading system universally accepted; diagnosis primarily clinical with imaging support 16.
  • Management

  • First-Line Treatment: High-dose parenteral thiamine (e.g., 500 mg IV daily) 1814.
  • Adjunctive Measures: Supportive care including hydration, nutritional support, and management of complications like hypothermia and infections 424.
  • Duration: Treatment duration varies; prolonged administration may be necessary depending on severity and underlying cause 8.
  • Special Populations

  • Pregnancy: Increased risk in hyperemesis gravidarum; prompt recognition and high-dose thiamine therapy crucial 519.
  • Elderly: Susceptible post-surgery, particularly cardiac surgery; vigilant monitoring and early intervention essential 7.
  • Comorbidities: Patients with schizophrenia or severe malnutrition also at risk; thiamine deficiency should be considered in differential diagnosis 20.
  • Key Recommendations

  • Initiate High-Dose Parenteral Thiamine: Administer 500 mg IV thiamine daily for suspected WE to improve clinical outcomes (Evidence: Moderate) 18.
  • Early Recognition and Prompt Treatment: Especially in high-risk groups like pregnant women with hyperemesis gravidarum and post-surgical elderly patients (Evidence: Moderate) 57.
  • Consider Thiamine Deficiency Beyond Alcoholism: Screen for and treat thiamine deficiency in patients with malnutrition, bariatric surgery, and prolonged parenteral nutrition (Evidence: Expert opinion) 31013.
  • Monitor for Complications: Address hypothermia, infections, and other complications alongside thiamine therapy (Evidence: Weak) 24.
  • Supportive Care Essential: Include hydration and comprehensive nutritional support alongside thiamine administration (Evidence: Expert opinion) 4.
