Injury of heart with hemopericardium

Last edited: 4/22/2026

Overview

Chylopericardium is a rare condition characterized by the accumulation of chyle (lymphatic fluid rich in triglycerides) within the pericardial sac, often secondary to thoracic duct injury or obstruction, though primary idiopathic cases exist 1311.

Diagnosis

  • Clinical Presentation: Asymptomatic or symptoms related to cardiac tamponade (e.g., dyspnea, chest pain, hypotension) 18.
  • Imaging: Chest X-ray showing enlarged cardiac silhouette, echocardiography revealing pericardial effusion 1811.
  • Fluid Analysis: Pericardiocentesis fluid analysis confirms chylous nature (lipid content > 10 mg/dL) 17.
  • Diagnostic Imaging: Enhanced CT and lymphangiography to identify thoracic duct abnormalities 610.
  • Nuclear Scans: 131I-triolein scan for noninvasive diagnosis of chylopericardium 12.
  • Management

  • First-Line Treatments:
  • - Dietary Modifications: Low-fat diet with medium-chain triglycerides 17. - Pericardiocentesis: Drainage of pericardial effusion 178.
  • Adjunctive Treatments:
  • - Octreotide: For refractory cases, particularly after failure of conservative therapy 1. - Surgical Interventions: - Pericardiectomy: Partial or total, with or without thoracic duct ligation 3456910. - Thoracic Duct Ligation: Direct surgical or thoracoscopic approach 45610. - Pericardial Window: Establishment for continuous drainage 6.

    Special Populations

  • Pediatrics: Commonly secondary to mediastinal anomalies like lymphangiomas; management includes surgical interventions 7.
  • Comorbidities: Intravenous drug addiction may be associated with nontraumatic chylopericardium, requiring surgical resection 9.
  • Key Recommendations

  • Perform pericardiocentesis for diagnostic and therapeutic purposes in suspected chylopericardium cases (Evidence: Moderate 17).
  • Consider surgical interventions such as pericardiectomy and thoracic duct ligation for refractory or recurrent cases (Evidence: Moderate 356).
  • Utilize octreotide as an adjunctive therapy in cases unresponsive to initial conservative management (Evidence: Weak 1).
  • Employ enhanced CT and lymphangiography for precise identification of thoracic duct abnormalities guiding surgical planning (Evidence: Moderate 610).
