Postoperative wound infection-deep

Last edited: 4/14/2026

Overview

Deep neck space infections (DNSI), particularly deep wound infections postoperatively, are serious suppurative processes involving deep visceral spaces of the neck, often requiring surgical intervention and antibiotic therapy 11628.

Diagnosis

  • Clinical presentation includes fever, neck swelling, dysphagia, and signs of airway compromise 116.
  • Imaging (CT, MRI) essential for delineating the extent of infection and guiding management 11727.
  • Laboratory markers such as elevated white blood cell count and neutrophil-to-lymphocyte ratio (NLR) can aid in diagnosis 1121.
  • Cultures from aspiration or intraoperative samples crucial for identifying pathogens and guiding antibiotic therapy 722.
  • Management

  • First-line treatment: Broad-spectrum intravenous antibiotics covering both aerobic and anaerobic bacteria (e.g., piperacillin-tazobactam, clindamycin) 2228.
  • Surgical intervention: Indicated for abscess drainage, especially in cases with airway compromise, extensive infection, or failure of medical management 11728.
  • Supportive care: Airway management, fluid resuscitation, and monitoring for complications such as sepsis 116.
  • Postoperative care: Close monitoring for signs of infection recurrence and timely follow-up imaging 30.
  • Special Populations

  • Pediatrics: Early surgical intervention is crucial for severe cases to prevent life-threatening complications 613.
  • Elderly: Higher risk of complications due to comorbidities; tailored management focusing on supportive care and close monitoring 14.
  • Comorbidities: Immunocompromised states increase susceptibility to severe infections, particularly with resistant organisms like Pseudomonas aeruginosa 214.
  • Key Recommendations

  • Initiate broad-spectrum intravenous antibiotics promptly upon suspicion of deep neck space infection (Evidence: Strong 22).
  • Perform imaging (CT/MRI) to assess the extent of infection and guide surgical intervention if necessary (Evidence: Moderate 17).
  • Early surgical drainage is recommended for abscesses causing airway compromise or extensive infection (Evidence: Moderate 117).
  • Monitor and manage potential complications, including sepsis and airway obstruction, closely (Evidence: Expert opinion 16).
  • Consider the patient's age and comorbidities to tailor management strategies, emphasizing early intervention in pediatric and high-risk elderly patients (Evidence: Moderate 614).
