Hepatorenal syndrome following delivery

Overview

Hepatorenal syndrome (HRS) following delivery is a rare but severe complication characterized by acute kidney injury in the context of liver disease, often seen in the postpartum period among women with pre-existing liver dysfunction or acute liver failure exacerbated by pregnancy.

Diagnosis

  • Clinical signs of acute kidney injury (elevated serum creatinine, oliguria) 27
  • Evidence of liver dysfunction (jaundice, coagulopathy, elevated liver enzymes) 27
  • Exclusion of other causes of kidney injury (e.g., pre-eclampsia, sepsis) 27
  • Diagnostic criteria such as the International Club of Ascites (ICA) classification for HRS 27

  • Supportive care including fluid management and renal replacement therapy (RRT) 27
  • Vasoconstrictive agents like terlipressin for hepatorenal syndrome type 1 (HRS-1) 27
  • Albumin infusion to improve renal function and hemodynamic stability 27
  • Management of underlying liver disease (e.g., antiviral therapy for hepatitis) 27

  • Pregnancy: HRS is uncommon but can occur in women with pre-existing liver disease; close monitoring and multidisciplinary care are essential 27
