Sciatica

Overview

Sciatica is characterized by radiating pain along the sciatic nerve distribution, typically below the knee, often due to lumbar disc herniation or spinal stenosis 20.

Diagnosis

  • Key Symptoms: Radiating pain below the knee, often unilateral, with possible associated numbness, tingling, or weakness 20.
  • Physical Examination: Positive straight leg raise test, femoral stretch test, and possibly a positive crossed leg raising test 34.
  • Imaging Studies: MRI is recommended for detailed visualization of disc herniations, nerve root compression, and other structural abnormalities 1730.
  • Electromyography (EMG): May be useful in cases where nerve root involvement is unclear 18.
  • Management

  • First-Line Treatments:
  • - Physiotherapy: Includes exercises, manual therapy, and modalities; shown to be effective in short-, medium-, and long-term outcomes 2. - Medications: NSAIDs for pain relief; muscle relaxants if muscle spasm is significant 12.
  • Adjunctive Treatments:
