Critical illness polyneuropathy

Overview

Critical illness polyneuropathy (CIP) is a syndrome characterized by progressive, predominantly motor, axonal neuropathy occurring in critically ill patients, often associated with multi-organ dysfunction and severe systemic inflammation. 1221

Diagnosis

  • Clinical presentation includes muscle weakness, particularly in the lower extremities, and diminished reflexes. 12
  • Electrophysiological studies (nerve conduction studies, electromyography) often show signs of axonal degeneration. 12
  • No specific diagnostic tests; diagnosis is clinical, often made in the context of severe systemic illness. 12
  • Management

  • Supportive care focusing on optimizing underlying critical illness management, including fluid balance, nutritional support, and control of inflammation. 1221
  • There is limited evidence for specific pharmacological interventions; trials with immunomodulatory therapies (e.g., intravenous immunoglobulin) show mixed results. 1221
