Sympathotonic orthostatic hypotension

Last edited: 4/23/2026

Overview

Orthostatic hypotension (OH) is characterized by a significant decrease in blood pressure upon standing, leading to symptoms such as dizziness, syncope, and increased fall risk, particularly in older adults 1810.

Diagnosis

  • Key Diagnostic Criteria: Systolic blood pressure drop ≥20 mmHg or diastolic drop ≥10 mmHg within 3 minutes of standing 20.
  • Recommended Tests:
  • - Orthostatic vital signs (supine and standing BP measurements) 20. - Tilt table testing for more definitive diagnosis in symptomatic patients 75.
  • Grading: Severity often classified based on symptoms and magnitude of BP drop 18.
  • Management

  • First-Line Treatments:
  • - Non-pharmacological: Increased salt and fluid intake, compression stockings, leg muscle exercises 112131. - Pharmacological: Fludrocortisone and midodrine for symptomatic relief 61347.
  • Adjunctive Treatments:
  • - Pyridostigmine for neurogenic OH 12. - Droxidopa (L-threo-3,4-dihydroxyphenylserine) for neurogenic OH 39. - Biofeedback and integrated rehabilitation programs 1532.

    Special Populations

  • Elderly: Higher prevalence and increased risk of falls; non-pharmacological interventions are crucial 1815.
  • Comorbidities: Consider impact on medication choices; caution with fludrocortisone due to higher hospitalization risk compared to midodrine 47.
  • Pediatrics: Limited data; focus on underlying causes and tailored interventions 56.
  • Pregnancy: Not specifically addressed in abstracts; individualized management required considering maternal and fetal safety [].
  • Key Recommendations

  • Perform orthostatic vital signs to diagnose OH, with tilt table testing for symptomatic patients (Evidence: Moderate 75).
  • Initiate non-pharmacological interventions such as increased salt and fluid intake, compression stockings, and leg exercises for symptom management (Evidence: Moderate 1121).
  • Consider pharmacological treatments like fludrocortisone and midodrine for symptomatic relief, weighing risks and benefits, especially hospitalization risk with fludrocortisone (Evidence: Moderate 647).
  • Tailor management strategies to the elderly population, emphasizing fall prevention and non-pharmacological approaches (Evidence: Moderate 18).
  • Evaluate and address potential drug interactions and comorbidities when selecting treatments for OH (Evidence: Expert opinion 47).
