Uncinate herniation

Overview

Uncinate herniation, also known as foraminal stenosis or lateral recess stenosis, involves the protrusion of ligamentum flavum or facet joint hypertrophy into the neural foramina, causing nerve root compression and symptoms similar to lumbar disk herniation, including radiculopathy and lower back pain. 16

Diagnosis

  • Clinical Presentation: Radiculopathy, lower back pain, and neurological deficits corresponding to affected nerve roots.
  • Imaging Studies: MRI is essential for visualizing the extent of neural foraminal encroachment and ruling out other causes 16.
  • Grading: No standardized grading system specifically for uncinate herniation; severity often assessed based on degree of foraminal narrowing and nerve root compression visible on imaging 16.
  • Management

  • Conservative Treatment: Physical therapy, nonsteroidal anti-inflammatory drugs (NSAIDs), and epidural steroid injections (e.g., transforaminal epidural steroid injection) can alleviate symptoms 11030.
  • Minimally Invasive Procedures: Percutaneous endoscopic lumbar discectomy (PELD) and endoscopic lumbar sequestrectomy may be considered for refractory cases 2528.
  • Surgical Intervention: Foraminotomy or laminectomy may be required in cases where conservative and minimally invasive treatments fail 13.
  • Special Populations

  • Pediatrics: Limited evidence; management typically conservative with close monitoring 23.
