Vestibular stenosis

Last edited: 4/14/2026

Overview

Vestibular stenosis refers to a narrowing or obstruction within the vestibular system, impacting balance and often presenting with symptoms like vertigo, dizziness, and gait disturbances. It encompasses various conditions affecting the inner ear and vestibular pathways. 611

Diagnosis

  • Clinical History: Detailed assessment of vertigo, dizziness, and associated symptoms. 6
  • Bedside Tests: Dix-Hallpike maneuver, head thrust test, and others to identify positional vertigo or other vestibular deficits. 1
  • Imaging: High-resolution CT or MRI to visualize structural abnormalities in the inner ear or skull base. 6
  • Vestibular Function Tests: Electronystagmography (ENG), vestibular evoked myogenic potentials (VEMPs), and caloric testing to assess function. 2026
  • Differential Diagnosis: Rule out other causes such as vestibular neuritis, migraine-associated vertigo, and Meniere's disease through comprehensive evaluation. 49
  • Management

  • Physical Therapy: Vestibular rehabilitation therapy (VRT) to improve balance and reduce symptoms. 5
  • Pharmacotherapy:
  • - Antiemetics: For vertigo management (e.g., prochlorperazine). 14 - Beta-blockers: In cases of associated hypertension or anxiety (e.g., propranolol). 10
  • Prophylactic Medications: For vestibular migraine, consider beta-blockers or calcium channel antagonists based on comorbidities. 10
  • Surgical Intervention: For specific cases of structural stenosis or obstruction, surgical options may be considered. 6
  • Special Populations

  • Pediatrics: Consensus guidelines recommend tailored vestibular testing protocols due to developmental differences. 2
  • Elderly: Increased vigilance for comorbidities affecting vestibular function and response to treatment. 17
  • Comorbidities: Management should consider interactions with existing conditions like migraine or cardiovascular disease. 1014
  • Key Recommendations

  • Comprehensive Otoneurological Evaluation: Include detailed history, bedside tests, and vestibular function tests for accurate diagnosis. (Evidence: Moderate 126)
  • Consider Vestibular Rehabilitation Therapy: First-line non-pharmacological intervention for symptomatic relief and functional improvement. (Evidence: Moderate 5)
  • Tailored Pharmacotherapy Based on Symptoms and Comorbidities: Use antiemetics for vertigo and consider beta-blockers for associated conditions like hypertension or migraine. (Evidence: Moderate 1014)
  • Avoid Unnecessary Tests and Treatments: Follow Choosing Wisely guidelines to minimize overutilization in vestibular disorders. (Evidence: Expert opinion 14)
