Fungal pneumonia

Last edited: 4/15/2026

Overview

Fungal pneumonia is an infection of the lungs caused by fungi, often presenting with severe complications such as cavitary lesions and systemic air embolism, particularly in vulnerable populations like the elderly 12.

Diagnosis

  • Clinical Presentation: Fever, respiratory symptoms unresponsive to antibiotics 1.
  • Imaging: Chest CT showing cavitary lesions, gas-fluid levels, and patchy opacities 1.
  • Exclusion Criteria: Rule out immune response, tumors, tuberculosis, and other systemic infections 1.
  • Specific Tests: Bronchoalveolar lavage, sputum cultures, and serological tests for fungal antigens 1.
  • Management

  • First-Line Treatment: Antifungal therapy, typically with voriconazole or echinocandins based on causative organism 1.
  • Adjunctive Measures: Supportive care including mechanical ventilation with caution to prevent air embolism 2.
  • Monitoring: Regular imaging and clinical assessments to evaluate response to treatment 1.
  • Special Populations

  • Elderly: Higher mortality, increased risk of misdiagnosis; consider fungal pneumonia in non-responsive cases 1.
  • Comorbidities: Patients with multiple organ failure or undergoing mechanical ventilation are at higher risk for complications like systemic air embolism 2.
  • Key Recommendations

  • Consider Fungal Pneumonia in Elderly Patients Not Responding to Antibiotics After Ruling Out Other Causes (Evidence: Moderate 1).
  • Exercise Caution with Mechanical Ventilation in Patients with Cavitary Fungal Pneumonia to Prevent Systemic Air Embolism (Evidence: Weak 2).
  • Initiate Antifungal Therapy Based on Susceptibility Testing or Broad-Spectrum Coverage When Fungal Infection is Suspected (Evidence: Expert opinion 1).
