Dry pleurisy

Last edited: 4/23/2026

Overview

Dry pleurisy, also known as pleuritic chest pain without significant pleural effusion, often results from inflammation of the pleura, typically due to viral or musculoskeletal causes. It is characterized by sharp chest pain exacerbated by breathing or movement 21.

Diagnosis

  • Clinical Presentation: Sharp chest pain, often worse with deep breaths or movement 21.
  • Exclusion of Other Causes: Ruling out pulmonary embolism, pneumonia, and cardiac issues through imaging and lab tests 21.
  • Imaging: Chest X-ray typically normal or shows minor changes; CT may reveal subtle pleural thickening 21.
  • Thoracentesis: Usually not indicated unless there is significant pleural effusion suspected 21.
  • Tuberculin Skin Test/IGRA: Consider in endemic areas to rule out tuberculosis 21.
  • Management

  • Conservative Measures: Rest, analgesics (e.g., NSAIDs), and avoidance of exacerbating activities 21.
  • Anti-inflammatory Therapy: Corticosteroids may be considered in severe cases or if there is suspicion of autoimmune etiology 21.
  • Monitoring: Regular follow-up to ensure resolution and rule out complications 21.
  • Special Populations

  • Pediatrics: Limited data; management similar to adults but with closer monitoring 21.
  • Elderly: Increased vigilance for underlying causes like malignancy or heart disease 21.
  • Comorbidities: Patients with respiratory conditions or autoimmune diseases may require tailored approaches 21.
  • Key Recommendations

  • Clinical Evaluation: Focus on detailed history and physical exam to differentiate from other causes of chest pain (Evidence: Moderate 21).
  • Imaging: Utilize chest X-ray initially; consider CT if initial findings are inconclusive or suspicion of underlying pathology remains (Evidence: Moderate 21).
  • Conservative Treatment: Initiate with rest and NSAIDs for pain management; escalate to corticosteroids if symptoms are severe or persist (Evidence: Expert opinion 21).
  • Close Monitoring: Regular follow-up is essential to ensure resolution and identify any complications early (Evidence: Expert opinion 21).
