Sewage workers' lung

Last edited: 4/14/2026

Overview

Sewage workers' lung, also known as hypersensitivity pneumonitis, is an occupational lung disease affecting workers exposed to organic dust, microorganisms, and irritants in sewage environments. It manifests as an inflammatory response leading to respiratory symptoms and potential chronic lung damage 1.

Diagnosis

  • Clinical history of exposure to sewage environments
  • Symptoms including cough, dyspnea, and fever
  • Physical examination may reveal crackles or signs of interstitial lung disease
  • Diagnostic tests: Chest X-ray, HRCT showing characteristic interstitial changes; pulmonary function tests showing restrictive pattern 1
  • Management

  • Removal from exposure environment
  • Corticosteroids for acute exacerbations (dose varies; individualized treatment)
  • Immunosuppressive agents in refractory cases (e.g., azathioprine, mycophenolate mofetil) 1
  • Special Populations

  • No specific guidelines provided for pregnancy, pediatrics, elderly, or comorbidities in the given abstracts 1
  • Key Recommendations

  • Ensure comprehensive documentation of occupational exposure and functional assessment in workers' compensation claims to support causation and facilitate return-to-work planning (Evidence: Expert opinion) 1
  • Implement preventive measures to reduce workplace incivility and violence, which indirectly supports overall worker health and reduces stress contributing to respiratory conditions (Evidence: Moderate) 3
  • Select evidence-based treatment guidelines for managing occupational lung diseases to standardize care and improve outcomes (Evidence: Moderate) 11
