Pulmonary transfusion reaction

Overview

Pulmonary transfusion reactions encompass acute respiratory distress following blood transfusions, often due to immune-mediated or non-immune mediated causes such as transfusion-related acute lung injury (TRALI) or allergic reactions.

Diagnosis

  • Clinical Signs: Acute onset of respiratory distress, hypoxemia, dyspnea, tachypnea, and potentially hypotension 1231.
  • Diagnostic Tests:
  • - Chest Imaging: Chest X-ray or CT showing pulmonary infiltrates 131. - Blood Tests: Elevated white blood cell count, specific IgE testing for allergies, and coagulation profiles 131. - Lung Function Tests: Arterial blood gas analysis to assess oxygenation and ventilation 131.
  • Differential Diagnosis: Distinguish from other causes of respiratory failure, such as sepsis or cardiogenic pulmonary edema 131.
  • Management

  • Immediate Actions:
  • - Stop Transfusion: Immediately halt the transfusion if a reaction is suspected 131. - Supportive Care: Administer supplemental oxygen, mechanical ventilation if necessary, and fluid management 131.
  • Specific Treatments:
  • - Antihistamines/Corticosteroids: For allergic reactions to alleviate symptoms 131. - Diuretics: To manage fluid overload if present 131.
  • Monitoring: Continuous monitoring of vital signs, oxygen saturation, and respiratory function 131.
