Gastroparesis syndrome

Last edited: 4/14/2026

Overview

Gastroparesis is a motility disorder characterized by delayed gastric emptying without mechanical obstruction, leading to symptoms such as nausea, vomiting, and postprandial fullness 14.

Diagnosis

  • Key Diagnostic Criteria: Symptoms include nausea, vomiting, early satiety, bloating, and weight loss; objective evidence required 14.
  • Recommended Tests: Scintigraphic gastric emptying study (GES) is the criterion standard; extend GES to 4 hours for pediatric patients to improve diagnostic accuracy 10.
  • Grading: Delayed gastric emptying confirmed by GES with standardized meal protocols (e.g., 99mTc sulphur colloid) 14.
  • Management

  • First-Line Treatments:
  • - Metoclopramide: Typically used, though caution due to risk of extrapyramidal side effects and tardive dyskinesia 211. - Domperidone: Considered for nausea and vomiting, monitor for QT interval prolongation when co-administered with QT-interacting drugs 8.
  • Adjunctive Treatments:
  • - Prucalopride: Off-label use for refractory cases; monitor for adverse events 2. - Haloperidol: As adjunct to conventional therapy in refractory cases, though evidence is limited to specific trials 7. - Dietary Modifications: Small, frequent meals, low-fat diet, and avoidance of high-fat and high-fiber foods 14.
  • Device and Surgical Interventions:
  • - Gastric Electrical Stimulation: For refractory cases 4. - Laparoscopic Gastrectomy: Considered for patients unresponsive to medical and other interventions 9.

    Special Populations

  • Pediatrics: Extend GES to 4 hours for more accurate diagnosis 10.
  • Comorbidities: Nutritional deficiencies may manifest as hair loss; multivitamin supplementation may be beneficial 3.
  • Key Recommendations

  • Use scintigraphic gastric emptying studies (GES) with standardized meal protocols for diagnosis (Evidence: Strong 114).
  • Initiate treatment with metoclopramide, considering potential side effects and contraindications (Evidence: Moderate 211).
  • Consider dietary modifications and adjunctive therapies like prucalopride for refractory cases (Evidence: Moderate 24).
  • Evaluate and consider gastric electrical stimulation or surgical interventions for patients with persistent symptoms despite medical management (Evidence: Expert opinion 49).
  • Monitor for nutritional deficiencies, particularly in patients reporting hair loss, and consider multivitamin supplementation (Evidence: Weak 3).
