Small bowel fistula

Last edited: 4/14/2026

Overview

Small bowel fistulas are abnormal connections between the small intestine and another organ or the skin, often complicating underlying conditions such as malignancies, inflammatory bowel diseases, or surgical interventions. 391011

Diagnosis

  • Initial Imaging: CT or MRI may identify fistulas but capsule endoscopy can provide detailed visualization of the small bowel mucosa. 1
  • Capsule Endoscopy: Recommended as first-line for suspected small bowel bleeding or obscure gastrointestinal bleeding. 1
  • Preparation: Patients should ingest a purgative (e.g., 2L PEG) before capsule endoscopy for better visualization. 2
  • Device-Assisted Enteroscopy: Useful for direct visualization and intervention in cases where capsule endoscopy is inconclusive. 167
  • Endoscopic Findings: Specific imaging features like dilated loops, air-fluid levels, and extravasation of contrast can indicate fistulas. 8
  • Management

  • Surgical Intervention: Often required for definitive closure of fistulas, especially those involving malignancy or severe complications like necrosis. 91011
  • Medical Management: Supportive care including nutritional support (e.g., enteral feeding with caution to avoid complications like necrosis). 9
  • Hemostasis: In cases of bleeding fistulas, endoscopic techniques (e.g., DBE with hemostatic agents) may be employed. 7
  • Anticoagulation/Thrombosis Management: Address underlying causes such as portal hypertension or thromboembolic events if applicable. 811
  • Special Populations

  • Pediatrics: Double-balloon enteroscopy (DBE) is feasible and safe for diagnosing and managing small bowel disorders in children, though specific fistula management protocols may vary. 6
  • Postoperative Patients: Caution with enteral feeding to prevent complications like small bowel necrosis, especially post-upper digestive tract surgery. 9
  • Key Recommendations

  • Use Capsule Endoscopy First-Line: For suspected small bowel bleeding, perform small-bowel capsule endoscopy as the initial diagnostic test due to its safety and visualization capabilities. (Evidence: Strong) 1
  • Patient Preparation for Capsule Endoscopy: Administer a purgative (e.g., 2L PEG) before the procedure to enhance visualization. (Evidence: Strong) 2
  • Outpatient Procedure: Whenever possible, conduct capsule endoscopy as an outpatient procedure to improve completion rates. (Evidence: Moderate) 2
  • Surgical Consultation for Definitive Treatment: For fistulas complicating malignancies or severe complications, surgical intervention is often necessary. (Evidence: Expert opinion) 91011
