Urate nephropathy

Overview

Urate nephropathy results from hyperuricemia leading to urate crystal deposition in renal tissues, causing acute or chronic kidney injury. It often manifests in patients with gout or undiagnosed hyperuricemia, potentially leading to renal impairment if not managed effectively 1622.

Diagnosis

  • Elevated serum uric acid levels 16
  • Presence of urate crystals in renal biopsies or urine sediment 16
  • Clinical signs of renal dysfunction (elevated serum creatinine, decreased glomerular filtration rate) 16
  • Imaging studies may show characteristic renal changes 16
  • Exclude other causes of renal injury through comprehensive metabolic panel and urinalysis 16
  • Management

  • First-line Treatment: Initiate urate-lowering therapy (ULT) with allopurinol or febuxostat to reduce serum uric acid levels 16
  • Adjunctive Measures:
  • - Hydration to promote uric acid excretion 16 - Corticosteroids or other anti-inflammatory agents for acute exacerbations 16 - Consider dialysis in severe cases with acute kidney injury 16
  • Monitoring: Regular follow-up to assess serum uric acid levels and renal function 16
  • Special Populations

  • Pediatrics: Limited specific data; ULT dosing adjusted for weight 122
  • Elderly: Increased risk of comorbidities; careful monitoring of drug interactions and renal function 16
  • Comorbidities: Consider impact on renal function and adjust ULT accordingly; close monitoring in patients with pre-existing kidney disease 16
  • Key Recommendations

  • Initiate urate-lowering therapy with allopurinol or febuxostat to target serum uric acid levels below 6 mg/dL in patients with urate nephropathy (Evidence: Strong 16)
