Nipple infection

Last edited: 4/10/2026

Overview

Nipple infection, often referred to as mastitis, is an inflammatory condition of the breast that can occur in lactating and non-lactating individuals. It is characterized by breast pain, swelling, and redness, and may be accompanied by fever and flu-like symptoms.

Diagnosis

  • Diagnosis is primarily clinical, based on signs and symptoms of inflammation 1.
  • Symptoms may include localized pain, tenderness, swelling, erythema, and warmth of the breast 1.
  • Systemic symptoms such as fever, chills, and malaise can also be present 1.
  • A breast abscess may develop as a complication, presenting as a fluctuant mass 1.
  • Management

  • First-line treatment:
  • * Antibiotics are indicated for suspected bacterial infection 1. * Commonly recommended agents include dicloxacillin, oxacillin, or cephalexin 1. * For penicillin-allergic patients, clindamycin or erythromycin may be used 1. * Treatment duration is typically 10-14 days 1.
  • Adjunctive treatments:
  • * Continued breastfeeding or milk expression from the affected breast is crucial 1. * Application of warm compresses to the affected area may provide symptomatic relief 1. * Analgesics such as ibuprofen or acetaminophen can be used for pain and fever 1. * Rest and adequate hydration are recommended 1. * If an abscess is present, surgical drainage may be necessary 1.

    Special Populations

  • No specific information on special populations was provided in the abstracts.
  • Key Recommendations

  • Antibiotic therapy should be initiated promptly for suspected bacterial nipple infection/mastitis 1. (Evidence: Moderate)
  • Continued breastfeeding or milk expression from the affected breast is essential during treatment 1. (Evidence: Moderate)
  • Symptomatic management with warm compresses and analgesics should be provided 1. (Evidence: Moderate)
