Overview

Sarcopenia is characterized by the progressive loss of skeletal muscle mass and function, often associated with aging and contributing to disability, frailty, and increased mortality 1414.

Diagnosis

  • Key Diagnostic Criteria:
  • - Low muscle mass (using criteria like appendicular lean mass index, psoas muscle cross-sectional area) 424 - Low muscle strength (handgrip strength, knee extension strength) 424 - Reduced physical performance (gait speed, Short Physical Performance Battery) 424
  • Recommended Tests:
  • - Dual-energy X-ray absorptiometry (DXA) for muscle mass 4 - Bioelectrical impedance analysis (BIA) 4 - Functional tests (gait speed, chair stands) 4
  • Grading:
  • - Mild, moderate, severe based on severity of muscle mass, strength, and performance deficits 4

    Management

  • First-Line Treatments:
  • - Nutritional Interventions: Protein supplementation (e.g., whey protein) 2922 - Physical Activity: Resistance training, aerobic exercise 3019
  • Adjunctive Treatments:
  • - Vitamin D Supplementation: To improve muscle function and strength 272136 - Deprescribing: Reducing polypharmacy to mitigate drug-induced sarcopenia 612

    Special Populations

  • Elderly: Higher prevalence and risk factors include polypharmacy, obesity, and vitamin D deficiency 121424
  • Comorbidities: Pain increases risk of sarcopenia 17, and dyslipidemia may be associated 7
  • Key Recommendations

  • Screen for Sarcopenia Using Established Criteria (Evidence: Moderate) 424
  • Implement Resistance Training Programs to Improve Muscle Strength and Mass (Evidence: Moderate) 3019
  • Consider Vitamin D Supplementation in Older Adults with Deficiency to Enhance Muscle Function (Evidence: Moderate) 2721
  • Evaluate and Optimize Nutritional Intake, Focusing on Protein Supplementation (Evidence: Moderate) 2922
  • Review and Potentially Deprescribe Unnecessary Medications to Mitigate Drug-Induced Sarcopenia (Evidence: Weak) 612
