Essential mixed cryoglobulinemia

Last edited: 4/14/2026

Overview

Essential mixed cryoglobulinemia is a systemic vasculitis characterized by the presence of circulating immune complexes composed of polyclonal immunoglobulins and monoclonal components, leading to multi-organ involvement including neuropathy, nephropathy, and vasculitic complications 234.

Diagnosis

  • Presence of mixed cryoglobulins (type II or III) in serum 13
  • Clinical manifestations indicative of vasculitis affecting multiple organs (e.g., neuropathy, renal involvement) 234
  • Biopsy evidence of vasculitis with vessel wall changes in affected tissues 3
  • Nerve conduction studies may show axonal sensorimotor polyneuropathy 3
  • Management

  • First-line treatments: Rituximab for immune complex reduction 1
  • Adjunctive therapies: Corticosteroids for acute exacerbations 2
  • Supportive care: Management of specific organ involvement (e.g., dialysis for renal failure, anticoagulation for thrombotic events) 4
  • Special Populations

  • Comorbidities: Careful monitoring and management of organ-specific complications (e.g., nephropathy, neuropathy) in patients with existing comorbidities 234
  • No specific data: Limited information on pediatrics, pregnancy, or elderly populations 1234
  • Key Recommendations

  • Monitor for development of cold agglutinin disease post-rituximab treatment, especially for signs of hemolysis and microvascular complications (Evidence: Weak) 1
  • Consider vasculitic involvement in peripheral and central nervous system symptoms, warranting nerve and brain biopsies for definitive diagnosis (Evidence: Moderate) 23
  • Evaluate for and manage cryoglobulinemic nephropathy with renal biopsy to confirm mesangiocapillary glomerulonephritis and associated vascular changes (Evidence: Weak) 4
